Acme Holdings LLC - IRS CP 575 A Authenticity Check: Request More (Fails 90-Day Rule)
Interpretation: Checked as of 5 October 2026. "Notice CP 575 A" is read as the original paper EIN notice in the CP 575 A-J series. The firm's rules are applied as given: issued within 90 days, name matches exactly. No image of the document was supplied, so every printed value is a user transcription.
Summary: The letter fails the firm's 90-day rule by a wide margin, and the printed address does not match the stated address (no suite). The EIN prefix is valid, but the register and the EIN itself could not be verified from public sources. Authenticity is neither confirmed nor refuted.
Field comparison
| Field | On document | Stated | Reference/register value | Match | Note |
|---|---|---|---|---|---|
| Entity name | Acme Holdings LLC | Acme Holdings LLC | Texas SOS record not retrieved (site returned 403; see Coverage) | unverifiable | Strings agree with each other; the name is generic and register-unchecked |
| EIN | 12-3456789 | 12-3456789 | No public EIN lookup found; prefix 12 is listed for Andover (IRS valid EINs) | match (document vs stated); EIN existence unverifiable | Sequential digits 1-9 (inferred red flag) |
| Address | 410 Pine St, Austin TX 78701 | 410 Pine Street Suite 200, Austin TX 78701 | none available | mismatch (suite missing) | "St" vs "Street" is an abbreviation; the missing suite fails an exact-match standard, which I applied to the address by analogy to the firm's name rule |
| Issue date | 03/14/2024 | not_provided | Today is 2026-10-05 | N/A | See recency check |
| Notice code | CP 575 A | not_provided | IRS describes an original CP575A-J series (IRS CP575 page) | unverifiable | Series exists; layout not compared |
| Form requirement | Form 1065 | not_provided | A domestic LLC with two or more members is a partnership for tax unless it elects corporate status (IRS LLC page) | consistent (inferred) | Implies multi-member LLC; membership unconfirmed |
| Issuer | Not transcribed | not_provided | none available (issuer assumed to be the IRS from the notice code; inferred) | unverifiable | Letterhead and return address not supplied |
Format and validity checks
- EIN format: pass (format only). The number has two digits, a hyphen and seven digits, and prefix 12 is listed for Andover (IRS valid EINs, accessed 2026-10-05). The IRS pages reviewed state no check-digit algorithm, so none was tested.
- EIN assignment: unverifiable. I found no public IRS lookup. Confirmation needs the IRS or a Letter 147C.
- Register status: unverifiable. The Texas Secretary of State page returned HTTP 403 to my fetch, and SOSDirect is its account-based service. No entity record was seen.
- Recency: fail. The letter is 935 days old (my calculation), and the 90-day window ended 12 June 2024.
- Name match: unverifiable against register. Document and customer strings are identical, but "exact" cannot be tested against the filed name.
- Address: fail (exact match) / unverifiable (existence). The suite is missing. I did not verify the street address with a postal tool.
- Document acceptability: pass in principle. The IRS says the digital CP575 is accepted by banks as written EIN confirmation, and the original CP575A-J cannot be duplicated or recreated (IRS CP575 page). My inference: an old original still records the EIN (the IRS page does not address the age of originals), but it fails this firm's recency rule. I did not review a regulator's KYC guidance on this document type.
Red flags
- High (verified, arithmetic on the transcribed date): Issue date is 935 days before today, far outside the 90-day rule.
- Medium (verified): The printed address omits "Suite 200", so the exact-match requirement is not met.
- Medium (inferred): EIN 12-3456789 is a straight ascending digit run, typical of placeholder numbers. This is my observation, not an IRS statement.
- Low (inferred): Generic name "Acme Holdings LLC" with no register match.
- Low (inferred): A Form 1065 requirement implies a multi-member LLC. Check the ownership structure.
Disposition
Request more. The letter is stale under the stated rule, the address differs, and neither the entity nor the EIN is independently confirmed. Escalate if the customer cannot supply the items below.
Request:
- A current IRS Letter 147C, or the digital CP575 from the customer's Business Tax Account (the IRS says it is a CP575A-J substitute).
- A Texas Certificate of Fact or formation certificate showing exact legal name, status and registered address.
- A proof of address showing "410 Pine Street Suite 200", or a written explanation of the missing suite.
Do next
- Run a Texas SOS entity search by name, then confirm by filing number and formation date.
- Have the customer send a fresh IRS confirmation directly from the IRS.
- Ask the customer to explain the address difference and give the LLC's member count.
Open items
- Whether any Texas entity named Acme Holdings LLC exists, and whether several share the name.
- Whether the address exists as a postal delivery point.
- The letterhead, return address, layout and fonts, against IRS samples.
Coverage note
The Texas SOS site blocked retrieval (403) and I did not use SOSDirect. IRS publishes no public EIN lookup that I found. No postal lookup, sanctions or adverse-media search was run. All sources accessed 2026-10-05.
