Drug Safety & Shortage Watch, US Pediatric Outpatient: Three Drug Rows in Active Shortage (29 Sep to 6 Oct 2026)
Interpretation: Regulator is the US FDA, with ASHP as the second shortage list. Window is 29 September to 6 October 2026. "GLP-1 receptor agonists" is expanded to agents seen on the FDA and ASHP lists (liraglutide, dulaglutide, semaglutide, tirzepatide, exenatide). Pediatric flags are marked [pediatric]. Brand names were not supplied, so no brand-to-generic mapping was needed.
Rows needing action: 3 of 4 supplied rows (amoxicillin, methylphenidate, GLP-1 class via liraglutide). The FDA Drug Safety Communications page I read was current only to 15 September 2026 and the recall list only to 1 October 2026, so neither covers the whole window; no listed drug appeared on either.
Act on this week
- Amoxicillin oral products (suspension, chewable, capsule, tablet), confirmed shortage on ASHP, bulletin updated 28 September 2026 (one day before the window opens, so inside the "this week" picture only as the current state): ASHP says Teva sold its beta-lactam plant to Chartwell and is discontinuing its beta-lactam line, including amoxicillin, over the coming months. ASHP lists Chartwell, Hikma, Rising and Sandoz as having oral presentations available, and says Aurobindo will not provide availability information. [pediatric] The amoxicillin/clavulanate oral bulletin was also updated 28 September, and says Hikma has 875 mg/125 mg tablets and several oral-suspension strengths on allocation.
- Methylphenidate extended-release tablets (FDA list, "Currently in Shortage", first posted 26 July 2023): the Lannett entry was reverified 1 October 2026, with the 27 mg (NDC 62175-311-37) and 36 mg (NDC 62175-312-37) tablets unavailable and estimated recovery November 2026. [pediatric]
- Liraglutide injection (FDA list, "Currently in Shortage", first posted 18 July 2023): Novo Nordisk's Victoza pens are on limited availability with estimated duration "TBD"; the Novo entry was reverified 22 September 2026. Saxenda is being discontinued in January 2027.

Notes
- Amoxicillin label postings: DailyMed page 1 shows three amoxicillin or amoxicillin/clavulanate SPL postings dated 29 September to 2 October 2026 (for example RemedyRepack and SportPharm). These are repackager postings. I did not open them, so I cannot say whether any wording changed; I treat them as no verified label change. Chartwell's multi-form amoxicillin SPL was posted 28 September 2026 (outside the window).
- FDA vs ASHP for amoxicillin: amoxicillin did not appear on the FDA shortage list text I retrieved; the confirmation comes from ASHP only (an ASHP-type list satisfies the confirmation rule).
- Methylphenidate: the ASHP bulletins were last updated 13 August 2026, so only FDA entries fall in the window. Besides Lannett, the FDA ER tablet page shows other manufacturer entries revised or reverified 1 October 2026, and a separate FDA shortage entry for methylphenidate extended-release film (patch); I did not review those. Several chewable, liquid and ER presentations carry old or mixed status on ASHP; check the specific product before acting.
- Safety and labels: the FDA Drug Safety Communications page is current only to 15 September 2026, so it cannot confirm anything for 29 September onward. Most recent items touching listed drugs are older: GLP-1 RA suicidal-ideation warning removal, 13 January 2026 and ER stimulant weight-loss labeling for children under 6, 30 June 2025. [pediatric] Neither is new this week.
- Methylphenidate and semaglutide DailyMed: the most recent methylphenidate SPL posting is 21 September 2026 (Rhodes ER capsule); the most recent semaglutide posting is 19 August 2026 (Novo Nordisk Ozempic/Rybelsus). Methylphenidate page 2 of results was not read. Neither shows an in-window posting.
- Recalls: the FDA drug recall list, current to 1 October 2026 and showing entries from 25 August onward, has no amoxicillin, methylphenidate or GLP-1 product; the 1 October entry is compounded glutathione, which is not a listed drug. MedWatch and the FDA enforcement report were not opened.
Do next
- Confirm local wholesaler stock of amoxicillin oral suspension and chewables by strength against the ASHP-listed makers.
- Check which methylphenidate ER strengths and brands your pharmacy partners can fill before November 2026.
- Open the FDA entry for Semaglutide Tablet to read the discontinuation date and scope.
- Read DailyMed labels for tirzepatide, exenatide and dulaglutide for any September 2026 revision.
- Open FDA MedWatch and the FDA enforcement report to close the recall check.
Coverage
Not reached or not opened: ASHP and FDA pages for tirzepatide, exenatide and lixisenatide; MedWatch; FDA enforcement reports; page 2 of DailyMed methylphenidate and amoxicillin results (amoxicillin 637 entries over 7 pages; methylphenidate 101 over 2 pages); the FDA shortage entry for amoxicillin (not seen on list). The ASHP site returned 403 to direct fetch; ASHP bulletin pages were read through a scraping fallback.
